Forma Pilates Studio Terms and Conditions

Forma Pilates Studio are dedicated to providing a safe, enjoyable, and effective Pilates experience. By booking a class or using our services, you agree to the following terms and conditions:

1.⁠ ⁠General Conduct

Respectful Behaviour: All clients must respect the rights and comfort of others. Disruptive behaviour or language will not be tolerated.

Health and Safety: It is the responsibility of each client to inform the instructor of any health conditions, injuries, or concerns prior to participating in class.

Age Requirements: Our classes are designed for individuals 16 years or older. Children under 16 may only attend with special permission and must be accompanied by a parent or guardian.

Personal Property: Forma Pilates Studio is not responsible for any lost, stolen, or damaged personal property.

2.⁠ ⁠Classes and Packages

Class Availability: All classes are subject to availability. We reserve the right to modify the class schedule and class times at any time.

Packages & Pricing: Prices are clearly outlined on our website or at the studio. Package prices are subject to change; however, previously purchased packages will be honoured at the price paid.

Expiry of Class / 1;1 Solo / 1;2 Duets Packages: Packages are valid for:

Single Session - 1 Month

Intro Pack - 1 Month

5 Lesson Pack - 2 Months

10 Lesson Pack - 3 Months

Valid from the date of purchase unless otherwise specified.

3.⁠ ⁠Studio Use

Dress Code: Clients must wear appropriate athletic attire for Pilates practice. Non-slip socks are recommended.

Equipment Usage: All studio equipment must be used as directed by instructors. If any equipment appears damaged or broken, please notify the studio staff immediately.

4.⁠ ⁠Medical Considerations

Health Declaration: Clients must complete a health questionnaire prior to their first class. It is important that you inform the instructor about any injuries, physical limitations, or health conditions (including pregnancy) that may affect your participation.

Medical Clearance: If you have any medical conditions or injuries that might impact your ability to participate in Pilates, you should consult a doctor before engaging in any class.

Refund Policy

At Forma Pilates Studio, we strive to ensure every client’s satisfaction. Our refund policy is as follows:

1.⁠ ⁠Class Passes and Packages

Non-Refundable: Once purchased, class passes and packages are non-refundable. Please review your schedule carefully before committing.

Injuries or Medical Conditions: If a medical condition prevents you from continuing with your Pilates classes, we may offer a credit toward future classes or packages, subject to a doctor’s note and approval from management.

2.⁠ ⁠Private Sessions

Refunds: Private sessions are non-refundable once payment has been processed. However, credits may be issued in cases of illness or emergencies with prior notice.

3.⁠ ⁠Errors in Payment

If an error occurs with a payment, please contact us immediately. We will resolve billing issues and process any necessary adjustments.

Cancellation Policy

We understand that life happens, but in order to ensure that our instructor can prepare and our studio remains efficient, we have a cancellation policy in place.

1.⁠ ⁠Class Cancellations / Rescheduling

Cancellation / Rescheduling Window: We require a minimum of 24 hours notice to cancel / reschedule a class booking. Cancellations / reschedules made after this window will be charged as if the class was attended.

No-Shows: If you fail to show up for a class without prior cancellation, you will be charged the full price of the session.

2.⁠ ⁠Private Session Cancellations

24-Hour Notice: Private sessions require 24 hours’ notice for cancellations. Cancellations made within 24 hours will be charged at the session rate.

Late Arrivals: If you arrive late for a private session, your session will still end at the scheduled time to avoid disrupting the next client.

Studio Etiquette

To ensure the best experience for everyone at Forma Pilates Studio, please observe the following etiquette:

Arrive Early: Arriving 5 minutes before your class starts allows you to settle in and get ready for the session. There maybe a 1:1 session happening if the doors closed please knock and wait

Phone Use: Please turn off or silence your phone during class to avoid distractions.

Personal Hygiene: Cleanliness is important in a group environment. Please practice good personal hygiene, including wearing clean, sweat-wicking clothes.

Noise: Keep noise to a minimum while others are in class. Respect others' space and focus.

Liability Waiver

By participating in any classes, workshops, or private sessions, you acknowledge and accept the inherent risks associated with physical exercise. You agree to release GRACE CAROLYN CARRETTA (Forma Pilates Studio) from any liability for injury or damages that may occur during class or while using studio facilities or equipment. A liability waiver must be signed before attending any session at the studio.
